Photo Consent Form

There are many activities for children and their families throughout the year at First Presbyterian Church, Shreveport.  In many cases, there will be photography at these events.  First Presbyterian Church, Shreveport will publish the photos from time to time for use on our website, church publicity, as well as in addition to various church print materials.  It is the church's policy that we do not use minors names when publishing their photos.  We ask that parents of minor children take the time to fill out the form.  Parents have to right to either give or decline permission for the church to use their children's photos.
